Common Techniques for Post-Mohs Reconstruction
Flap techniques
This is the most commonly used Mohs reconstruction technique and involves borrowing nearby skin and fat as a unit, or a flap, and transferring the defect. Often, these “flaps” may need to be staged and will require two to three surgeries to complete the reconstruction.
Cartilage grafting
Cartilage may need to be borrowed from the ear or rib to provide support in places such as the nose.
Bone grafting
Bone may be taken from the skull, hip, or jaw in order to provide support.
Tissue expansion
If there is not enough adjacent or nearby tissue, then expanding areas of the face may be required. This is in a small percentage of people and is similar to the process of tissue expansion most commonly seen in breast reconstruction after mastectomy.
Skin grafts
Skin grafts may be required if there are limited options for reconstruction or as a part of a staged approach.
Anesthesia
Anesthesia requirements will depend on the size of the defect and the type of reconstruction that may be required. Surgery may be able to be performed under local anesthetic while the patient is awake. Most times, it requires the patient to be placed under general anesthesia with the aid of an anesthesiologist.
Timing
The lesion or skin cancer will be removed by the Mohs dermatologist. Typically, the lesion can be closed the same day by the dermatologist. If the patient is referred to a plastic surgeon, then your reconstruction may take place in a couple of days. This is often due to operating room availability at a surgery center or hospital. It is acceptable for reconstruction to take place within one week’s time.
Appearance
Regardless of the techniques used, plastic surgeons will take care, whenever possible, to position the resulting scar along the natural creases and curves of your face. It helps minimize and camouflage the appearance of your scar. In addition, follow-up surgeries may be required to improve the appearance of the scar, such as dermabrasion—“sanding” down of the scar to improve contour.
Aftercare
You will be observed for a while after your surgery has been completed, and then given instructions for returning home. Some people, especially those who have a flap procedure, may be given a prescription for antibiotics to take at home. Smoking
One of the most important things you can do for yourself and to maximize the outcome of your surgery is to not smoke. Smoking, through many mechanisms, can decrease blood flow, increasing your chance of infection and wound healing issues.
